A fee of $5 must accompany the FIPPA Access Request Form. Please include a cheque or money order payable to Woodstock Hospital or provide credit card information on page 2 of the Form. Please do not put cash in the mail.
Additional fees apply to access requests as dictated by the legislation and are the responsibility of the requestor. The Requestor will be contacted if the request exceeds $100.00 to confirm the requestor’s wish to proceed with the request. The requestor may be required to pay half of the anticipated cost prior to the FOI office proceeding.

FIPPA Fees
The hospital is permitted to charge certain fees when responding to a FIPPA access request. The following fees will be charged.
General Information
- Application Fee (mandatory) – $5.00
- Photocopies, computer printouts and scanning – $0.20 per page
- Records provided on CD-ROMs – $10.00 for each CD-ROM
- Manually searching a record – $7.50 for each 15 minutes spent by any person
- Preparing a record for disclosure, including severing a part of the record – $7.50 for each 15 minutes spent by any person
- Developing a computer program or other method of producing a record from a machine readable record – $15.00 for each 15 minutes spent by any person
- Costs, including computer costs, incurred in locating, retrieving, processing and copying the record(s) if those costs are specified in an invoice received by the hospital as per received invoice.
