Patient and Family Advisory Council

About Us

We believe the best care is achieved when patients, families, and caregivers are actively involved in decision-making. The Patient & Family Advisory Council (PFAC) is a collaborative group that includes patients, family members, caregivers, and hospital staff who work together to enhance care by ensuring our design and delivery of services meet their needs and the values set out by the Woodstock Hospital Declaration of Patient Values.

We are committed to fostering a diverse membership that reflects our community. By embracing different perspectives, we aim to ensure that our decisions and recommendations are inclusive and representative of all those we serve.

Roles & Responsibilities

  • Advise and share information
  • Provide opinions as they relate to hospital programs and services
  • Act as Woodstock Hospital ambassadors in the community
  • Support people-centred care
  • Participate as partners in the planning and operations of specific programs within the PFAC mandate
  • Review and provide feedback on policies, processes, patient education materials, and communication strategies

Eligibility

Members must be a patient, family member, or caregiver of a patient who has received care at Woodstock Hospital and lives or works in the Oxford County region.

Commitment

  • Three-year term
  • Attend PFAC meetings and join other councils and committees throughout the hospital
